Workers’ Comp injuries, provide claim number, ICD-9 codes for allowed condition, letter from the Bureau certifying allowed diagnosis, date of injury and address where injury occurred.   Health Care Plans Most major health insurance plans are accepted. To verify coverage of a Horizon physician, you can contact your health insurance carrier or call our patient accounts department at (216) 265-8007.   Emergency/ After Hour’s Procedures Should a medical emergency arise after normal business hours, dial any of our office locations and the call will ring through to the answering service. If your medical situation is life threatening, please go to the nearest emergency room.   Appointment Scheduling Information Call any of our offices during normal business hours to schedule an appointment.
